NASA released a report on Boeing’s (BA) Starliner mission that kept two astronauts stuck on the International Space Station for nine months, providing details on communication breakdowns as well as the “unprofessional behavior” as the agency and contractor struggled to agree how to return the crew to Earth, Joey Roulette of Reuters reports. “Starliner has design and engineering deficiencies that must be corrected, but the most troubling failure revealed by this investigation is not hardware,” NASA Administrator Jared Isaacman wrote in a letter to NASA employees. “It is decision making and leadership that, if left unchecked, could create a culture incompatible with human spaceflight.”
