PointClickCare Unveils AI Discharge Intel to Tighten Post-Hospital Care Coordination

PointClickCare has introduced Discharge Intel, an AI-driven solution designed to give health plans access to clinical intelligence within 24 hours of a patient’s hospital discharge, aiming to close a critical information gap in transitions of care. Launching at HIMSS 2026, the product extends PointClickCare’s role as an interoperability hub connecting acute, post-acute, and payer stakeholders on a single network to reduce readmissions, avoidable costs, and care fragmentation.

The company will demonstrate Discharge Intel alongside real-world customer use cases at the conference, highlighting results from major health systems that have leveraged its network—now used by more than 30,000 provider organizations and all major U.S. health plans—to streamline cross-setting workflows. PointClickCare will also showcase its interoperability capabilities and customer outcomes, including a case study from the University of Maryland Medical System reporting a 32% reduction in skilled nursing facility-related readmissions over two years, underscoring the platform’s potential financial and operational impact for payers and providers.
