Evergreen Nephrology has launched Evergreen Connected Care, a new AI-enabled kidney care management service built on a strategic partnership with Jaan Health’s Phamily platform, positioning the company to expand its value-based kidney care capabilities nationwide. The service combines Evergreen’s clinical expertise and network of nephrologists with Phamily’s AI-driven care management tools to help practices deliver population-wide, proactive care for patients with chronic kidney disease across all stages and care transitions.

By embedding Evergreen clinical staff on top of Phamily’s platform, Evergreen Connected Care aims to strengthen fee-for-service revenue streams today while laying the operational groundwork for future value-based contracts and risk-bearing models. The offering supports enhanced care management, omni-channel patient communication between visits, and more reliable reimbursement for frequent, out-of-clinic care coordination that historically has been undercompensated.

Chief executive officer Heather Trafton said the model is designed to relieve administrative and financial pressures on nephrology practices by offloading high-touch care management to Evergreen, allowing nephrologists to focus more time on in-person patient care. The platform’s AI capabilities support clinical decision-making, build comprehensive care plans, and automate follow-ups and documentation, which can increase throughput without proportionally increasing staffing costs.

For patients, Evergreen Connected Care provides ongoing access to a multidisciplinary team and structured support as they move through CKD stages, dialysis transitions, and related comorbidities, which can reduce gaps in care and potentially lower avoidable hospitalizations. Jaan Health’s CEO, Nabeel Kaukab, noted that Phamily’s decade-long clinical AI track record and workflow automation infrastructure are being integrated directly into Evergreen’s national nephrology network, providing practices with standardized enrollment, documentation, compliance, and performance management capabilities.

Operating across 24 states with more than 1,000 providers, Evergreen Nephrology can deploy Evergreen Connected Care as a scalable, plug-in solution for its existing and future partner practices, strengthening its competitive position in value-based kidney care. The initiative is expected to support higher-margin, proactive care models by improving billing for between-visit services and lowering overall care costs, which may enhance Evergreen’s economics in both shared-savings and fully risk-bearing arrangements. For payors and other stakeholders, the move signals Evergreen’s intent to use AI and centralized care management as core levers to drive outcomes, patient satisfaction, and sustainable financial performance in a complex, high-cost patient population.
