The ENLIGHTEN-4 Phase 3 program from Eli Lilly and Co. tests a new weekly shot, eloralintide, in adults who have knee osteoarthritis pain and are overweight or have obesity. The goal is to see if this drug can ease chronic knee pain safely over time, a space where current options are limited and long term use often brings side effects.

Eloralintide, also known as LY3841136, is given as a simple under the skin injection once a week. The drug aims to reduce knee pain tied to osteoarthritis in a way that may be easier to maintain than pills or more invasive treatments, while a placebo arm helps show how much benefit comes from the drug itself.

The study uses a randomized, double blind design, so people are assigned by chance to eloralintide or placebo and neither patients nor doctors know who gets which. The trials run in parallel under one master protocol, and the main goal is treatment, making the data directly relevant for future approval and for real world use if the drug succeeds.

The study was first submitted on Jan. 16, 2026, signaling the formal start of regulatory and site setup work. The most recent update was filed on Feb. 9, 2026, which shows the trial plans are active and being refined as Lilly prepares to move from planning into participant enrollment and long term follow up.

As a Phase 3, not yet recruiting program, this update is early but important for investors watching LLY. A successful knee pain drug for people with obesity would expand Lilly’s musculoskeletal and metabolic reach beyond diabetes and obesity and could pressure rivals in pain and inflammation, though any readthrough to earnings remains years away.
