The cardiovascular kidney and metabolic (CKM) health assessment and patient empowerment trial, officially titled “Cardiovascular Kidney and Metaboolic (CKM) Health Assessment and Patient Empowerment in chROnic Disease Using a Health Coach INtervention ModEl: A Randomized Clinical Trial,” tests whether AI tools and remote monitoring can cut heart, kidney and diabetes risks in chronic disease patients.
