Evergreen stressed the importance of team-based care, including coordinated clinical support and access to social and emotional resources for patients. Management positions these services as tools to improve education, clarify medications, manage symptoms, and ultimately reduce avoidable hospitalizations under risk-based contracts.

Key clinical metrics included a 58% optimal start rate for kidney replacement therapy, delayed progression to kidney failure in 35% of a monitored stage 4 chronic kidney disease cohort, and a transplant access rate 25% above the national average.
